This form is for individual players seeking Football NSW sanction approval to participate in the 2023 Viking Challenge hosted by Great Lakes United Football Club. Please note that to receive approval all players must be registered for the 2023 winter football season with a Football NSW Club, complete this form and upload a letter of support from their 2023 football club.

    I declare that I am a current 2023 registered player with a Football NSW Club on the Football Federation Australia National Registration Database. I have sought and obtained the approval and support from my 2023 Football Club to participate in the 2023 Viking Challenge and am not currently suspended by my Club, Association or Football NSW. I confirm all details provided in this sanction application form are accurate and I understand that providing incorrect or misleading information will lead to my sanction application being withdrawn, therefore meaning I will not be covered by any Football NSW insurance policies.
