The following insurance information is provided by Football NSW (FNSW) for FNSW registered participants and affiliated clubs and associations.
PERSONAL ACCIDENT INSURANCE
The Personal Accident Insurance provides Capital Benefits and other claimable benefits for up to 12 months from date of injury to supplement non-Medicare medical expenses, weekly loss of income (max. $250 per week) and student tutorial fees which arise out of injuries during participation in official football activities sanctioned by FNSW including overseas travel during sanctioned representative tours, subject to policy terms and conditions.
Non-Medicare medical expenses include ambulance costs, private hospital accommodation, physiotherapy, chiropractic, dental, theatre fees in private hospital, orthotics, splints, prosthesis and hire of artificial aids. Physiotherapy benefits are capped at $350, however where surgery for an injury is required, physio relating to that injury is claimable up to the $5000 maximum non-Medicare Medical Benefits section. LODGE A CLAIM

Claims are lodged online via the Gow-Gates Football Federation Australia claims portal. If you have not lodged a claim previously you will be required to Sign up and create a Username and Password which will allow you to lodge the details of your claim and then continue to access the portal to view the progress of your claim and upload any required documentation and your receipts for reimbursement. (Note: The claims portal is not the FFA ‘Play Football’ registration portal, so your username and password for ‘Play Football’ will not enable you to access this portal)
During the claims process you will also be required to download and print a ‘Club Declaration’ form and a ‘Physician’s Statement’ form. The Club Declaration is required to be completed and signed by your club to confirm that you are a registered member of the club and that your injury occurred during official training or games or other sanctioned (approved) club activity. The Physican’s Statement is required to be completed by your Doctor or Specialist to confirm the details of your injury, or your Dentist where dental treatment is required. Both forms are uploaded into the portal once completed.
Claimable benefits for Non-Medicare medical are paid to the claimant as a reimbursement. Ambulance costs are paid for by the person named on the NSW Ambulance invoice. The NSW Ambulance receipt (once the invoice is paid) is then uploaded with your claim or after your claim has been lodged for reimbursement.
CERTIFICATES OF CURRENCY
If your club requires a Certificate of Currency (CoC) email your CoC requirements to [email protected]
Include in the email:
- The name of your club
- Your club’s contact person
- The email you require the CoC to be sent to
- The name of the relevant Council, if your local Council asks that it be named as the “Interested Party” on the CoC document.
E.g. Please name the “Interested Party” on the CoC as Hills Shire Council
