Spectator misbehaviour;

Issues in regards to the facility (lighting or change rooms)

Postponements, abandonments, forfeits; or

Serious injuries (for example where an ambulance attends).

A few tips on how to write your incident report:

Be precise and accurate

Remember the key elements:

Who was involved?

What did they do or say exactly (use the 1st person)?

Where did it occur and where were you at that time?

Did you see/hear the incident or was it reported to you?

Keep your report short and straight to the point! Point form is sufficient.
