Prior to submitting this Notice of Intention to Play, please carefully read the Football NSW Gender Diversity Policy (Policy) and the Explanatory Note to the Football NSW Gender Diversity Policy both of which are available here. If you have any difficulties using this form, please contact integrity@footballnsw.com.au. Please note you are only required to submit this Notice of Intention to Play if you:

play, or intend to play, Football in a Relevant Competition during the 2024 season;
are aged 15 years or over; and
were not assigned female Sex at birth.

Compulsory supporting documents*

At the time of lodging with FNSW a Notice of Intention to Play, you must also lodge records of your concentration of testosterone in serum levels for the period of 24 months prior to the date of lodgement of this Notice of Intention to Play. Note:

Your latest test must be dated no more than three months prior to you lodging this Notice of Intention to Play.
The Expert Panel may, at its discretion, not take into account any measurements of serum testosterone levels lodged with FNSW that were not conducted by means of tandem mass spectrometry.
