I have sought and obtained the approval and support from my current Football Club to participate in the 2025 Viking Challenge and am not currently suspended by my Club, Association or Football NSW. I confirm all details provided in this sanction application form are accurate and I understand that providing incorrect or misleading information will lead to my sanction application being withdrawn, therefore meaning I will not be covered by any Football NSW insurance policies.
