Consent and Release

1. I, the participant listed (or the Parent/Guardian of the participant listed), consent to Football NSW Limited (ACN 003 215 923) (FNSW) and any person authorised by them to:
(a) record my (and/or my child’s) name, image, likeness, voice and signature (the Images) by any means, in any format and on any media; and
(b) change, copy, edit, publish, broadcast, distribute and communicate the Images to the public by any means, in any format and on any media and for all other current and future purposes authorised by FNSW including publicity, promotional, advertising and educational activities of FNSW and its partners, notwithstanding that such use may infringe any moral rights I (and/or my child) may have within the meaning of the Copyright Act 1968 (Cth).

2. I acknowledge that the consent given above is genuinely given and is not given because any person:
(a) applied duress to me to give that consent; or
(b) made a false and misleading statement to me in relation to the giving of that consent.

3. I further acknowledge and agree that ownership of all intellectual property rights in the Images will, upon their creation, vest in and be the sole and exclusive property of FNSW.

4. I further acknowledge and agree that I am not (and/or my child is not) entitled to any payment for any use or exploitation of the Images and that I relinquish any right that I(and/or my child) may have to examine or approve the completed products where my(an/or my child’s) Image may be included intact or in part.

5. I also release FNSW from all claims and rights I (and/or my child ) may have or claim to have or, but for this consent and release, might have had against FNSW in relation to its use of the Images, or the use by any person authorised by FNSW.
